Did you undergo a medical procedure at the Assuta Ashdod Hospital? For the sake of your health, we meticulously keep the information about your procedure, your test results, and the medical documentation you received. A special hospital department is tasked with managing and organizing our patients’ medical records, ensuring that this documentation is carefully stored, and enabling its convenient use in future if needed.
Yaakov Atrakchi, Department Manager, explains: “We open both a physical file and a digital file for each new patient, where we store all medical information about the procedure the patient undergoes. Doctors use that information for treatment and followup. It can also be used by the patient and/or by other medical staff who care for the patient in future. The job of our Department is to collect, preserve, and control the hospitalization files at the hospital. We also perform ongoing audits to maintain the quality of our data retention.”
Patients and their authorized representatives, HMOs, attorneys and courts who require medical documentation or information such as surgery reports, discharge letters, and the like, may contact our department. After properly submitting a request using the appropriate form and in accordance with Ministry of Health regulations, they can receive the medical information stored in the Department.
